Diabetic Myonecrosis Mimicking Acute Compartment Syndrome of the Thigh

چکیده

Diabetic myonecrosis is an underreported complication of long-standing, poorly controlled diabetes mellitus [1]. It affects both type 1 and type 2 diabetic patients, is usually self-limiting and responds well to conservative management [1-4]. It commonly affects the lower limb with thigh muscles being most commonly involved [1,3]. The disease commonly affects women (61.5% of all cases), type 1 diabetes patients (59% of all cases) and patients with long-standing diabetes (mean duration of disease 14.3 years) [2]. Although it most commonly involves a single limb, bilateral involvement has been described in 8.4%of cases [2]. Since the condition presents with characteristic symptoms of compartment syndrome [1-3], it can easily be misdiagnosed and treated surgically. Operative intervention increases morbidity and prolongs recovery of the patient [1-3,5]. We report a case of diabetic myonecrosis presenting with symptoms of compartment syndrome and its management.

We report a case of sudden thigh pain from spontaneous quadriceps necrosis, also known as diabetic myonecrosis, in a 28-year-old patient with poorly controlled diabetes mellitus. Diabetic muscle infarction is a rare end-organ complication seen in patients with poor glycemic control and advanced chronic microvascular complications. Proposed mechanisms involve atherosclerotic microvascular occlus...

Diabetic myonecrosis: likely an underrecognized entity.

Diabetic myonecrosis is a rare complication of long-standing diabetes mellitus that presents as acute onset of swelling and pain of the affected muscles. The differential diagnosis includes cellulitis/pyomyositis, necrotizing fasciitis, neoplasm, and deep venous thrombosis (DVT). Missed diagnoses can lead to unnecessary invasive diagnostic procedures and inappropriate treatment. The diagnosis i...

Acute compartment syndrome of the thigh 10 days following an elective primary total hip replacement.

Acute compartment syndrome (ACS) of the thigh is an uncommon condition usually treated surgically by emergency dermofasciotomy. We report a rare case of acute delayed compartment syndrome of the anterior compartment of the thigh following an uncemented Total Hip Replacement (THR). Surgical decompression was performed and patient had full recovery.

Acute compartment syndrome of the thigh after joint replacement with anticoagulation.

We describe three patients with a compartment syndrome of the thigh, two after total hip replacement and one after total knee replacement. Two of the patients were fully anticoagulated. A compartment syndrome of the thigh is a rare, but important complication of joint replacement surgery if patients are receiving anticoagulants. Close observation is needed and when indicated monitoring of the i...

Treatment of compartment syndrome of the thigh associated with acute renal failure after the Wenchuan earthquake.

Compartment syndrome of the thigh is a rare emergency often treated operatively. The purpose of this study was to evaluate the effects of nonoperative treatment for compartment syndrome of the thigh associated with acute renal failure after the 2008 Wenchuan earthquake. Nonoperative treatment, which primarily involves continuous renal replacement therapy, was performed in 6 patients (3 men and ...
